Elevated Wellness Clinical Services provides individual therapy to individuals ages 18 and up. I provide timely, effective, and research based interventions to help clients achieve their goals. All individual therapy is currently 100% virtual.

Rates

Rates and Insurance for Individual Therapy - 

Rate - $150 for 50 minutes session. For cash pay clients, a limited number of sliding scale spaces are available. Please reach out to inquire on availability. 

Insurance - Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. If you choose to use insurance, you will be routed through Headway, who handles insurance benefits and billing. All client must create an account with Headway to review insurance coverage.

I accept the following insurance: Aetna, Anthem Blue Cross and Blue Shield, Carelon Behavioral Health, Cigna, Quest Behavioral Health, and Select Health - Colorado

Payment - I accept PayPal and Zelle as forms of payment.

Cancellation Policy - Every client is provided with one free cancellation per 6 month period for any reason. After that, there is a $60 cancellation fee. If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you will be charged $100 for the session. You may reschedule two times, up to 24 hours before your appointment without incurring a cancellation fee. 

No Show Fee - If you do not reschedule or cancel your appointment by prior to the appointment start time, you will be charged a no show fee of $150.
